Complete the form below to request a VitaHemp wholesale account. All fields are required unless marked optional. Your Minnesota LPHE-R license will be verified against the OCM registry before your account is approved.

What you'll need before you start:

  • Your Minnesota LPHE-R license number and expiration date
  • A copy of your LPHE-R license (PDF or photo)
  • Your licensed business address
  • The email address you want to use for your wholesale account
